I. General Rules

No purchase is necessary to enter or to win. A purchase will not improve the chances of winning.

Participation in the contest constitutes entrant's full and unconditional agreement to, and acceptance of, these official rules and regulations. Winning a prize is contingent upon fulfilling all requirements set forth herein.

The contest period begins 12:00:00am CENTRAL
time in the United States on October 27, 2007, and closes 11:59:59pm CENTRAL
time in the United States on September 1, 2008. To be eligible, entries
must be completed and received on or by the closing date and time. |

The grand prize winner will receive a GE Healthcare
Oncology Workstation [latest commercially available platform including AdvantageSim
MD, with multi-modality and multi-phase capability, Advantage4D (workstation
software only) and additional software as determined by GE Healthcare].
Four 2nd place winners will receive a $200 (USD) gift certificate for www.anatomical.com.
*NOTE: The grand prize will ONLY be awarded IF 10 complete entries are
received by September 1, 2008. |

The grand prize winner has the obligation to report the prize on their cost report to the government to ensure there are not any false claims. Also, to use the GE Healthcare Oncology Advantage Workstation® in a reimbursable service, the HCP must have reported the acquisition and the cost of acquisition. Winners are solely responsible for all taxes and/or fees that may be incurred. Winners will be notified at ASTRO 2008.

Winners will be announced at the ASTRO 2008 Annual meeting on Monday at 1:00pm in the GE Healthcare booth, unless otherwise specified.

Judging will occur during the three weeks before ASTRO 2008. The judges' decisions are final and binding on all matters relating to this Contest. The odds of winning depend upon the total number of entries received. Prizes are not transferable or assignable. No substitution or cash redemption allowed except by GE Healthcare, who may substitute a prize of equal or greater value if advertised prizes become unavailable. By accepting prizes, winners consent to the use of name, state of residence and likeness for advertising and GE marketing promotional purposes online and in print, or in any other media worldwide in connection with the Contest, without limitation and without additional compensation worldwide, except where prohibited by law.

The contest is open to facilities utilizing GE Products. Images submitted must have been acquired on the GE LightSpeed or the GE Discovery family of CT or PET / CT scanners, and must showcase one of the following applications: AdvantageSim MD or Advantage4D.

All submissions must be made in English.
|
| 10. |
All applicants must be the legal age of majority in their country, province or state of legal residence.
|
| 11. |
GE Healthcare and the Contest Committee reserve the right to alter any contest rules at anytime. In the event a contest committee member is not available, another person will be assigned to fill their position.
|
II. Submitting Case Studies

Applicants must electronically sign the customer release form as prepared and accepted by GE Healthcare acknowledging acceptance and understanding of the complete contest rules.

The Customer Release Form must be electronically
signed on or before September 1, 2008. |
| 14. |
All submissions must be accompanied by acknowledgement of patient consent and de-identified, complying with the most recent version of the HIPAA regulations (HIPAA citation for image de-identification: 45 CFR 164.514(a)-(c).). Any entries received that do not meet de-identified requirements will be disqualified and immediately discarded.

Patient subjects must be notified that their de-identified images and clinical information will be distributed consistent with an executed Patient Release Form.
|
| 16. |
A representation of GE Products is required.
Images submitted must have been acquired on the GE LightSpeed or the GE

following applications: AdvantageSim MD or Advantage4D. |

For each case submitted, you may submit no more than four (4) images.

| 18. |
Multiple entries from the same contestant are permitted. Each entry must be submitted entirely separate from any other entry. Each institution may only be awarded (1) prize. In the event of qualifying for more than (1) prize, an institution will be awarded the prize of greater value.
|

The prize will be awarded directly to the institution from which the case study was obtained and not to any individual.
|
| 20. |
The preferred image format is bitmap. Images must be in one of the following formats: jpeg, gif, tiff, bmp, or png. The maximum file size per image is (2) MB. Avi or other video formats will not be accepted for contest consideration.

Entries must be submitted electronically via the contest webpage link. Entries received by telephone, fax, courier or personal delivery will not be considered.
|
III. Selection Criteria

Images will be judged on the following criteria:
a. Clinical Relevance: Treatment planning impact and patient outcome
b.Overall Quality of Case Presentation: Sufficient clinical detail, written description of case, image quality, visual presentation, and overall impact of treatment planning
c. GE Product Representation : Demonstrated clinical utility or benefit of scanner or post-processing application.

The panel of judges will be selected by the GE Healthcare Contest Committee to include representation from global academic, clinical, and technical radiation oncology institutions and departments.
|
| 24. |
A panel consisting of (6) - (10) judges, who are not contestants or have an interest in the contest outcome will be chosen to review entries.
a. If less than (10) entries are submitted, all judges will review all cases.
b. If more than (10) entries are submitted, all judges will review (6) - (10) randomly assigned cases. The top (10) cases will be re-reviewed by all judges in a second round.
c. In the event of a tie, a winner will be chosen from the tied entries in a drawing at the award presentation.
d. No entry placed before the judges will identify the contestant.

IV. Additional Rules

GE Healthcare has the right to use and distribute any and all images collected during the contest internally and externally pursuant to and limited by HIPAA regulations and patient consent.
|

The Contest Committee reserves the right, at its sole discretion, to cancel, terminate or suspend the Contest should any unauthorized intervention or other cause beyond the control of the Contest Committee affect the proper administration or conduct of the Contest.
|
| 27. |
GE Healthcare assumes no responsibility for illegible, lost, delayed, misdirected or incomplete entries, system errors and failures, faulty transmissions, or other telecommunications malfunctions.
|
| 28. |
The submitter will not be notified of an incomplete or rejected entry.
|
