GE Healthcare Customer Release
I consent to the reproduction of the case images and associated case information for use in posting of clinical cases on the GE Healthcare website, and/or for softcopy and hardcopy prints to be used by the GE Healthcare sales and marketing force.
I acknowledge that the patient is aware his/her images have been submitted to GE Healthcare as part of a contest. The patient has completed and signed a release form that is available upon request by a GE representative for verification.
I understand any data or images received from the site with identifiable patient descriptors will be disqualified and discarded.
This shall be a continuing consent and applies to all case information provided now and in the future, per the intended purposes, with no limitations or reservations.
Reference will be made to the site supplying the case information.
